Please download the below PDF, filling in all the applicable information to apply for Special Seismic Certification through VMC Group's engineering services division. Upon submission of the signed application and receipt of the listing fee, the process of obtaining a VMA Number will begin.
For those who have testing completed by another organization or have self-certified, please fill out all sections.
